Premier Physical Therapy proudly performs all billing in-house. Our billing coordinators are available to assist patients with any billing or insurance questions they may have.

Premier Physical Therapy accepts a large variety of plans. Please reference the list below for common plans we are contracted with. Please note, this in not a comprehensive list. If you are unsure if we are considered an in-network provider with your insurance, please contact your insurance directly to verify network status.

At this time, Premier Physical Therapy does not accept auto insurance claims and does not bill on a lien.

Premier Physical Therapy does not bill out-of-network plans; however, we will be happy to provide patients the necessary documentation required for reimbursement from their insurance if out-of-network benefits apply.

If you choose to schedule with Premier Physical Therapy and plan to utilize your insurance, you must provide all current health insurance information at or before the initial visit. It is the patient’s responsibility to update Premier Physical Therapy if their insurance coverage changes at any point during treatment. Our billing department does not bill retroactively or “back-bill”, and therefore any services rendered during a lapse or change in coverage may be subject to our self-pay rate.

Once scheduled, our billing department will conduct an insurance verification on your behalf to determine an estimated cost per visit prior to beginning your treatment. If your physical therapy benefit has a copay, the copay will be collected upon check-in for each visit. If your physical therapy benefit is subject to your deductible, you will be billed for services once the claim has been processed by your insurance.
